Request for School Records
Requests for certified copies of the Ministry of Education Transcript of Grades or the Permanent Student Record Card can be made through the Records Management Department, provided that the last public school attended within the Province of BC was in North Vancouver School District.
Schools have the authority to correct errors and omissions. If the school is not in session or has since closed, the Records Management Department may be able to assist you with your request.
At the end of July, regardless of graduation status, the Ministry issues a transcript to all Grade 12 and adult education program students who have successfully completed at least one Grade 12 course.
Schools and post-secondary institutions designated by students on Post-Secondary Institution (PSI) Choices Forms automatically receive an official printed or electronic copy of the student's transcript from the BC Ministry of Education.
Obtaining your Personal Education Number (PEN)
The Personal Education Number (PEN) is a nine-digit number assigned to each student enrolled or registered with any; B.C. Early Learning (Strong Start program), Public or Independent school, B.C. Certified Offshore School, Yukon School, or public Post-Secondary Institution.
Your nine-digit PEN can be found on your student records or report card.
To obtain your Personal Education Number (PEN), please contact the Ministry of Education, Obtaining your PEN or complete the Request for School Records. A legible copy of government issued photographic identification must accompany the request form. Please do not send originals.

Transcripts
A transcript is a listing of a student's academic record relating to a BC Graduation Program, which includes courses completed successfully, grades earned, honours received, and graduation status.
Your transcript, not your Graduation Certificate (diploma), is the legal document proving high school graduation.
Official Transcripts, Graduation Certificates and GED records can be obtained from the Ministry of Education in Victoria B.C. online at Ministry of Education - Transcripts and Certificates.
Any questions or concerns about ordering official documents from the Ministry of Education can be emailed to student.certification@gov.bc.ca.
The Ministry of Education Student Transcript Service will have your transcript if you attended a BC or Yukon Secondary school and you:
- Graduated after 1985
- Completed a grade 12 course after 1986
- Attended grade 12 prior to 1974 in the University Entrance or Academic-Technical Program
If you graduated in 1985 or earlier, you can obtain your transcript by completing a Request for School Records.
Is there an error or omission on your transcript?
Please contact your last school of record when in session, or the Records Management department, with details of the error or omission and include all of the following information:
- The student's legal name and maiden name, if applicable
- The student's date of birth
- The student's Personal Education Number (PEN), if known
- Your contact information; i.e., phone/cell numbers and mailing address
- The name of the school in North Vancouver School District that the student last attended
- The date (month and year) that the student last attended the school
- Course details, including date and school at which course taken

Permanent Student Record (PSR) Cards
A Permanent Student Record Card (PSR) is a listing of a student's entire academic record for grades K-12, which includes courses taken, achievement records, absences, schools attended and graduation status. A student's educational record follows the student as she/he progresses through the British Columbia public school system. Therefore, it is important to know the name of the last school the student attended. More information: BC Ministry of Education Permanent Student Record Order.

Please request a copy of your Permanent Student Record Card by completing a Request for School Records.
Letter of Attendance Package
A Letter of Attendance Package includes a letter detailing the dates within which a student was registered to attend the North Vancouver School District, the name(s) of the school(s) attended, a certified copy of the Permanent Student Record (PSR) Card, and conformation of the parent/guardian names and the home address for the student currently on file.
Requesting a Letter of Attendance Package
Please complete and submit the Request for Student/School Information Letter. A legible copy of government issued photographic identification for the student (and parent/guardian, if student under the age of majority) must accompany the request form. Please do not send originals.

Completing a Request for School Records
If you wish to request your records for your own personal use, such as an employment application or registration at another educational institution, please complete the Request for School Records Form.

SUBMITTING THE FORM
Your completed Request for Student Records form may mailed to Records Management, Educational Services Centre, 2121 Lonsdale Avenue, North Vancouver, BC, V7M 2K6 or e-mailed to records@sd44.ca (please read carefully the privacy alert related to email communications on the form).
SERVICE LEVEL
You will be contacted within one week of receipt of your records request to inform you of the status of your request. During winter, spring and summer vacation periods it may take longer to process requests.
PROVISION OF STUDENT RECORDS
Ensure that you have specified how you would like to obtain your student records on the Request for Student Records form.
Records can be
- E-mailed (please read carefully the privacy alert related to email communications on the form)
- Mailed (cost of postage will be added to the fee associated with the request)
- Collected in person
PAYMENT
- Payment for student records must be received prior to records being released.
- Once the availability of your records has been confirmed you will be contacted by North Vancouver School District staff to request payment.
Unclaimed student records ordered for pick up will be destroyed 3 months after they are issued. No refunds will be issued should the records not be picked up.
Consent to Release
If you wish to grant access to your records to a third party, such as a law firm, ICBC or other agency, please complete and submit the Consent to Release Information to a Third Party Form.
