Consent Forms:
Please complete the following consent forms through School Cash On Line (KEV) (there is no cost associated with these forms):
- Personal Information Consent, Parent and Student consent is required
- Impromptu Walking Field Trip
- Student Acceptable use of Technology Agreement
Student Information Verification Form:
Verification forms are sent home with all students at the start of the school year. Please update contact information for you/family directly on the form. If your home address has changed, please provide us with the following:
Required Documents for Address Updates:
Proof of address in B.C.
Accepted documents for home owners:
- Purchase Agreement - new home purchase with subject removed
OR
- Current utility bill (BC Hydro, Fortis, landline telephone or internet/cable statement).
Accepted documents for renters:
- Formal rental agreement AND a current utility bill (BC Hydro, Fortis, landline telephone or internet/cable statement) in either the landlord(s)'/homeowner(s)' or the tenant(s) name (as per the lease/tenancy agreement).
- Note: if utility bill is in the landlord's name please also provide a 3rd document in the renter's name such as a bank statement, BC vehicle registration, renter's insurance or Gov't issued ID.
**Return signed forms to the office even if there are no changes**
If your address changes during the school year, please contact the school office for a verification form to be sent home with your student.

Medical Alerts:
The Medical Alert guidelines have been updated as of September 2022, for more information please check HERE.
There are students in school who may have a physician diagnosed, potentially life-threatening medical condition, and may require emergency care by school staff. This is referred to as a Medical Alert Condition. The purpose of this document is to provide parents/guardians, students, administrators and school staff with guidelines for the development of the Medical Alert List and for managing the emergency plans for life-threatening medical conditions. Parents/guardians, students and staff all have a shared responsibility for keeping students safe at school.
Please complete a Medical Alert Plan(s) if your student has any of the following conditions, forms are available HERE.
- Anaphylaxis
- Asthma
- Diabetes
- Seizures
- Other Conditions Requiring Emergency Care
Please submit your medical alert plan(s) to the Handsworth School office as soon as possible.
Absences:
Student Extended Absences for family holidays (5 or more days) or for other non-medical reasons.

Athletic Registration Forms:
Students who wish to participate on a Handsworth School Team are required to complete 6 Athletics Registration Forms, and return them to their Coach:

- Student Awareness of Risk
- Informed Consent
- Student Medical Form
- Athletic Citizenship
- Concussion Awareness Sign off Sheet

Field Trips Consent Forms:
Walking Field Trip consent forms are completed on-line through your student's Cash-on-Line Account.

Students who wish to submit applications to be considered to be Handsworth's nominee(s) for external awards should regularly check the Grad 2025 Teams page
If you would like to be considered for a school-based award, you can find the application form in the Scholarship Info channel of your CLC Team. Applications close on Sunday, February 2, 2025 at 11:45 pm. Late submissions will not be accepted, and only students who complete the application form will be considered for awards.
