​​​Congratulations on having been selected to represent the Seycove Seyhawks! 

There are 5 key steps that need to be taken to complete your registration for the upcoming season. 

Follow these steps, print the appropriate pages and return them to your Athletic Director/coach.  You are ineligible to compete in any sport until your coach and/or the Athletic Director has each of the forms and athletic fees. 

This process should take approximately 10 minutes to complete. Please follow the instructions below:

1. Click on the appropriate sport link below. Read the Informed Consent form, print the document, complete the form, and return to the Athletic Director/Coach. 
2. Complete the Seycove Student Athlete Code of Conduct. Print the document, complete the form, and return to the Athletic Director/Coach.
3. Complete the North Vancouver School Distict Medical Form. Print the document, complete the form, and return to the Athletic Director/Coach.
4. Complete the Student Awareness of Risk and Responsibility.  Print the document, complete the form, and return to the Athletic Director/Coach.
   
5. Athletic fees must be paid online via SchoolCash.Net. Students are not eligible to play unless all fees are paid.  Please contact Mr. Cardle if assistance with fees is needed at pcardle@sd44.ca
