Parent Consent Forms/Cash On-Line

Dear Parent/Guardian,

The North Vancouver School District is pleased to announce that three parent consent forms are officially online through our existing School Cash Online system. The following three forms are available on School Cash Online as of September, 2018.

    -  Personal Information Consent
    -  Impromptu Walking Field Trip Informed Consent
     -  Student Acceptable Use of Technology Agreement

Consent for all other forms is still provided on paper and you will receive these from your child's school. These paper forms must be completed and returned to the school. 

Deadline for submission of all forms, including the three online forms, is: September 21, 2018

• To register you will need your child's North Vancouver School District student number, which appears at the top of this email. Please note that you have to use your child's full legal last name when registering (also located at the top of this email).
• If you are completing the forms for more than one child, please check the name on the top left corner of the form to ensure you are completing it for the correct student.
