September 2024-25 Start Up - Student Forms Required

It's very important that these forms are completed as soon as possible so your student can access the programs needed to complete school work, your student's photo can go in the yearbook and on the website and they can participate in activities off the school grounds.

  • Impromptu Walking Field Trip form
  • Student Acceptable Use of Technology Agreement
  • Personal Information Consent (don't forget to select Secondary as shown below)

Fees will be available for payment online effective October 1st, 2023.  Instructions to register for online payment are available at the above link.

Medical Alerts

The Medical Alert guidelines have been updated as of August 2020, for more information please check HERE.

If your child has a "Medical Alert" please complete the forms below and return to the Main Office, fax to the school at 604.903.3601 or scan a copy to windsor@sd44.ca.  A supply of his/her medication and food (e.g. snacks and medication, Benadryl, EpiPEN etc), can be kept in the office in case of emergency or as needed or kept with the student. Please indicate on the form where the medication will be kept for our records.

Absences (Extended Absence) Form

Fee Waiver Form

Please complete the ​​Application for Waiver of School Fee 2024-25.pdf and return it to the office to the attention of the Principal


​​Student Parking Form

           Students driving to and parking at school must register their vehicle with the main office.

Students are welcome to park in the parking lot off Lytton Street but not in any of the spots outside the Preschool or Dance school.

​Windsor Service Credit        

           Service hours are unpaid service provided to the school and are not associated with course credit.
